Early Intervention Team

The Early Intervention Team is made up of Physiotherapists, Occupational Therapists and Therapy Assistant Practitioners.

The team works across the Emergency floor including our Emergency Department (Accident and Emergency), Emergency Assessment and Discharge Unit and the Short Stay Medical Unit.

The main population served by the Early Intervention team is older adults with a variety of acute conditions including, but not limited to :

Falls

Fractures

Confusion

Decreased mobility

Frailty

The service aims to assess a persons mobility and ability to engage in their activities of daily living, providing individual interventions to support people when they leave hospital.

We liaise with our community colleagues to provide rehabilitation and reablement in the community to maximise their functional potential within their own environment.

As the Team is involved at such an early stage of a person's hospital admission, we play a vital role in preventing avoidable admissions by enabling supported discharge.

88% of patients referred to EIT were discharged form EADU, 83% of patients referred to EIT were discharged from A&E .
