We are listening to our residents, staff and communities across Great Yarmouth and Waveney to understand what people think about health and social care services in the community.
Community Services covers health and care provision that happens outside of a hospital, delivered by social care, primary care, council services, public health and voluntary, faith, and social enterprise sector groups.
Community services are mainly delivered in people’s homes (including care homes) but also in community hospitals, clinics and schools. Community health teams play a vital role in supporting people with complex health and care needs to live independently in their own home for as long as possible. Services also include health promotion services, such as school health services and health visiting services.
Many services involve partnership working across health and social care teams, made up of a wide variety of professionals including community nurses, allied health professionals, district nurses, mental health nurses, therapists and social care workers.
We want to ensure your thoughts and views are firmly embedded into describing what the future vision for community services across Great Yarmouth and Waveney should look like, and how we can improve outcomes and experiences for both residents and staff. This feedback will form part of a review of community services across the whole of Norfolk and Waveney.
You can give your views by attending an online workshop. The Great Yarmouth and Waveney event takes place on Tuesday 1 August 2023, 5-6pm (please register to secure your place)
In addition, a short survey is available to capture the views and thoughts of people, and is open until Sunday 12 August 2023.
