Meet the team
Sarah Hay, Dementia Care Liaison Nurse
Sarah supports patients and their carers while in hospital and also provides training for staff.
Kim Crosswell, Patient Pathway Co-ordinator
Kim helps to identify patients with dementia or memory difficulties and tracks them during their stay.
