Children's Community Nursing Team
We are a five day a week service seeing children and families in their own home, schools, colleges or nurseries.
We will see any child from birth to their 19th birthday with a nursing need; this includes any child with complex medical and nursing needs with disabilities, long term sick children such as those with diabetes, epilepsy, arthritis, cancer or any other long term condition. We also see children and families with short-term issues such as stitch removal, dressing change or giving certain medicines.
The Children’s Community Nursing Team (CCNT) offer training to children and their families as well as teachers, carers & other medical or nursing staff. We generate and update school health care plans allowing children with medical and nursing need to attend school.
Some specialist services are offered such as the Diabetic Nurse Specialist, Epilepsy Nurse Specialist, & Neonatal Nurse Specialist, ADHD & ASD specialist practitioners, and a one-stop Allergy service where children can be seen, diagnosed and treated in one session.
Within CCNT there is a Respite service. This offers children and families with sickness or special needs a few hours a week break when a professional carer will look after the child in their own home while the rest of the family have a well earned break.
CCNT also offer some specialist clinics such as Healthy Weight and Enuresis (bed wetting).
We have a Clinical School/Community Nurse based at The Warren & John Grant Schools.

How can I get a referral?
If your child requires the service of the CCNT your GP, hospital consultant, nurse, school nurse, health visitor, teacher, social worker or any other professional involved with your child may refer.
If you are concerned that your child may need the services of the CCNT please speak to any of these professionals who can refer to us. We will assess all new referrals within 24 hours of receiving the referral and be in touch with the child and family.
