Our Disclosure Log information has been published where the Trust feels it is appropriate and where it has been considered that the information may be of public interest.

The Disclosure Log is divided into categories below. 

If you are unable to find the information you are looking for you can make a Freedom of Information request by emailing FOI@jpaget.nhs.uk

However, before making a request, please ensure that you have searched our Disclosure Log and Publication Scheme, as well as the wider website, as the information may already be available.

Please be aware that any requests submitted may be made available on the website as part of our Disclosure Log but all personal details about the requester will be removed prior to the response being published.
