Disclosure log

Our Disclosure Log is a list of responses to previous Freedom of Information (FOI) requests which have been released to the public. We aim to routinely publish our responses to requests made under the Freedom of Information Act 2000 as part of our commitment to being honest and open.

The Freedom of Information Act 2000 allows information about the Trust, the services it provides and how it operates to be disclosed to the public. In our Disclosure Log information has been published where the Trust feels it is appropriate and where it has been considered that the information may be of public interest.

The Disclosure Log is divided into categories below. Within these categories the responses are arranged in alphabetical order via the nature of the FOI query. Please click on the yellow text to access the FOI response. 

If you are unable to find the information you are looking for you can make a Freedom of Information request by emailing foi@jpaget.nhs.uk . However, before making a request, please ensure that you have thoroughly searched our Disclosure Log and Publication Scheme as the information may already be available.

Please be aware that any requests submitted may be made available on the website as part of our Disclosure Log but all personal details about the requester will be removed prior to the response being published.
